
Invitation to join CVRM education programme
Practice nurses working at an advanced practitioner level are invited to join an educational programme aiming to improve the management of people with diabetes, cardiovascular disease and chronic kidney disease.
Elevate is a clinical leadership programme for lead nurses in primary care to equip them to learn, grow and improve the care of patients with cardiovascular-renal-metabolic (CVRM) conditions.
More than 5 million people in the UK have been diagnosed with type 2 diabetes, at least half of whom have CKD. Type 2 diabetes is one of the main risk factors for premature death from CVD.
Elevate will comprise a two-year programme of meetings and webinars, led by expert speakers and advisors, including Practice Nurse editor in chief, Beverley Bostock.
